You have now decided that you would like to apply for supported housing and you need to complete an application form. You will also need to complete a medical questionnaire if you have any medical conditions and/or disabilities.
When submitting your application form you will also need to provide certain documentation which is listed on the application form.
You can obtain an application form and medical form by:
