If you have a disability or a long-term medical condition and are finding it difficult to carry out everyday activities in your home, like going up and down the stairs, getting on and off your armchair or using the bath, we can offer an assessment by an Occupational Therapist or Occupational Therapy Assistant.
Our aim is to help people of all ages (from children to adults) to be as independent as possible in their everyday lives.
Depending on the type and level of help you need we can provide a range of services. These include:

The advice and assessment is free of charge. The equipment is free and usually on loan for the period required.
If you are a Council Tenant, adaptations to the property are free.
If you are a Housing Association tenant, they make their own funding arrangements.
Major adaptations in privately owned or rented properties are funded by applying for a Disabled Facilities Grant.
Please note that we do not reimburse equipment purchased or minor and major adaptations privately installed.
Our Occupational Therapists and Assistants are based at the Community Disability Service at the address shown.
Our technicians fit equipment and we use contractors to complete minor and major adaptations.
At present, all requests for services must be made through the Adult Intake and Access Team for adults or to Children and Families Assessment Team for children.
If you prefer, you can ask a friend or relative to make contact on your behalf.
The person making the referral will be asked about your contact details and the nature of the difficulties you are experiencing, including:
This information enables the Adult Intake and Access Team or Children and Families team to assess whether you are eligible for our help or whether there is another more appropriate service for your requirements.
You can also complete the Contact Assessment Form for older people.
You can also ask your GP to refer you to our service.
If you are having in-patient treatment at any of the local hospitals, you will receive a service from the hospital Occupational Therapy service.
If you are already have an allocated care manager, who is a Trusted Assessor he or she might be able to request equipment or minor adaptations following their assessment. If not they can then make a referral to this team for a specialist assessment.
One of our Occupational Therapists of Occupational Therapy assistants will make an appointment to visit you by telephone or letter.
During the visit you will have the opportunity to explain and demonstrate the difficulties you experience.
The assessment may take an hour or more, depending on the problems that need to be discussed. If you wish, you can ask a relative, friend or carer to be with you when we visit.
During the visit the Occupational Therapist or Assistant will ask you about your medical condition and social support. They will ask you to demonstrate certain activities for example, how you manage the stairs, access the front and back door, getting on and off the toilet, chair and bed and in and out of the bath.
The worker will discuss possible solutions with you and arrange for the services to be provided following the home visit.
If the provision of equipment or minor adaptations is agreed, it will be delivered or fitted within 7 working days.
Once the equipment and minor adaptations have been provided, the Occupational Therapist or Assistant will visit you again to demonstrate how to use it safely.
Major adaptations take longer to complete and the worker will discuss the time frames with you.
We always welcome your views on our service, therefore please complete the 'Complaints, comments and compliments' leaflet provided by the worker during their initial visit.
