Community Learning Disability Team

 

The Community Learning Disability Team assess individual needs and arranges appropriate care for adults (aged 18 to 60 or 65 years) with a wide range of learning disabilities.

 

We currently provide support and care to over 600 service users within the borough. Any assistance provided would aim to promote an individual's ability to remain as independent as possible.

 

What does the service provide?

Who is eligible for the service?

Service users must have a diagnosis of learning disabilities and be registered as a learning disabled person and be a residents of the borough.

 

How is the service accessed?

You can make your own referral for an assessment, or someone else who knows you well can make a referral for you (although you must agree to being referred and be aware that you are being referred). Telephone the Community Learning Disability Team at the number shown. Alternatively contact the Adult Intake and Access Team.

 

All referrals are processed through the Referral and Allocation Meeting for initial visit for a joint assessment.

 

Is there a charge for the service?

Means test to certain services i.e. home care packages. No charge for health service.
