The Physical and Sensory Impairment teams are committed to providing and developing services that respect an individual's right to choice and promoting independence and inclusion.
We use the term 'impairment' instead of the word 'disability', to highlight our commitment to the social model of disability.
There is a range of services available to residents in the borough who have a physical or sensory impairment and need help with their day to day care needs.
Services which can be provided include specialist advice and information, assessment of need, packages of care for those people who are eligible, support for carers and to people who are HIV positive or have an AIDS diagnosis.
An example may be day care, respite care or services to help at home. You may also need equipment to help with everyday tasks or adaptations to your home to help you get about more easily.
If you or someone you know may need support from the services we provide, you should contact the Physical Impairment Screening Service at the address shown. Alternatively you can complete an online referral form.
Once a referral is accepted you will be contacted by a member of our team who will talk to you about what sort of help you might need. They may also talk to anyone who currently helps you at home.
This will lead to an assessment, which will help us to decide which services we can arrange for you. Following this assessment, a care plan is agreed, setting out what services are to be provided.
Not everyone who asks us for help can be given a service because we must make sure we can help those in greatest need. In order to obtain a service, you will have to meet our eligibility criteria.
If we cannot arrange a service, then we may refer you on to the most appropriate agencies. For example, this could be Age Concern or Citizens Advice Bureau.
There is no charge for the assessment. There is a small charge for most community care services like meals in the home or home care. Residential or nursing care can also cost quite a lot. Your social worker will explain any costs to you.
We hope this information will begin to explain the processes involved and of course the social worker will explain any of these matters in more detail when they meet you.
