Adult Placement Scheme

 

Who is the service provided to?

The Adult Placement Scheme provides placements in carer's homes for adults who are registered as having a learning disability. The service user will have been assessed by Learning Disabilities Care Management as requiring either a short-term or long-term placement.

 

Once placed, it is expected that the person will treat the house as his or her own home. They will have their own bedroom and access to all other rooms in the house and will live as a member of the family. The Adult Placement Scheme is used for long term; short term and respite placements.

 

Age range

18 - 64 years.

 

Who is eligible for the service?

Commonly, the service user must be registered as having a learning disability and have an up-to-date care plan and core assessment. Care Management will make a referral and subject to staff/placement availability and following a relevant matching process, a service will be offered.

 

How is the service accessed?

By referral from the Learning Disabilities Care Management team. Following referral, the service user will meet directly with an allocated case manager who will oversee an agreed package of support and provide sessional support as required, as stated in a service agreement.

 

Is the receipt of the service time-limited?

No. However, the service is needs-led. Reviews will be held and service provision will reflect the individual service user's needs.

 

Capacity

The Adult Placement Scheme has carers who cater for service users requiring long-term placements, as well as those who offer planned short-term placements or emergency respite placements. Carers are recruited on a needs-led basis, although assessment is thorough and can take up to 6 months.

 

Is there a charge for the service?

People who live in an adult placement are required to contribute towards the cost of their placement. This charge will be supplemented by the borough, with a rate agreed following a financial assessment of the service user's income.

Not applicable.

 

Opening hours

The office is staffed 9am - 5pm, Monday to Friday (excluding bank holidays).

 

Support staff may work outside these hours, dependent on the specific needs of the service user and the availability of the sessional staff.
