Premises
This deals with premises who wish to carry out licensable activities.
Application for a Premises Licence to be granted under the Licensing Act 2003

This application form is required when a premises wishes to carry out licensable activities like:
- The sale by retail of alcohol
- The provision of regulated entertainment, which include:
- A performance of a play
- An exhibition of a film
- An indoor sporting event
- A boxing or wrestling entertainment (indoor and outdoor)
- A performance of live music
- Any playing of recorded music
- A performance of dance
- Entertainment of a similar description to that falling within the performance of live music, the playing of recorded music and the performance of dance
- The provision of late night refreshment
Application to vary a Premises Licence Word 737K
This application form is required when a premises who is already licensed wishes to vary any of the times and or their licensable activities.
Application to transfer a Premises Licence Word 277K
This application form is required when a premises changes ownership.
Application to vary a Premises Licence to specify a designated premises supervisor Word 188K
This application form is required when a Designated Premises Supervisor is changed from one person to another.
Consent of premises licence holder to transfer Word 107K
This form is required to be completed when an application is made for a transfer premises licence.
Form of consent given by the person whom the applicant wishes to be the premises supervisor Word 115K
This form is required to be completed by the person whom the applicant has named as the person to become the Designated Premises Supervisor.
Notification of an interest in Premises Word 265K
This form is required by anyone who has a legal interest in the premises but who may not be the applicant.
Application for a provisional statement to be granted under the Licensing Act 2003 Word 731K
This application is used for anyone who wishes to gain a provisional statement to premises which are:
- Being or about to be constructed for the purpose of being used for 1 or more licensable activities, or
- Being or about to be extended or otherwise altered for that purpose (whether or not they are already being used for that purpose)
Form of consent given by the person who holds the existing licence Word 123K
This form is required when there is a change of licence holder at the premises and must be signed by the current licence holder.
Interim authority notice under the Licensing Act 2003 Word 207K
This form is required when the former licence holder has either died, become mentally incapable or become insolvent.

Reviews of Premises Licences and Club Premises Certificates
The application form is required by anyone wishing to seek a review of a licence that has been previously granted under the Licensing Act 2003.
A review must be based on 1 or more of the licensing objectives these are:
- Prevention of crime and disorder
- Prevention of public nuisance
- Protection of children from harm
- Public safety
