You may ask us to start a statutory assessment.
If you have concerns about the progress your child is making, always talk first to the teachers at the school. You will be able to discuss your child's progress and any other action that can be taken in school to meet any particular needs that have been identified.
If your child makes very little progress after specialists from outside the school have been involved, a review meeting will be needed. At this point, we might decide to recommend a statutory assessment. The special educational needs coordinator at school will fill in a form providing important information that we would need before agreeing to an assessment. Request forms (

It is essential that you include your child's name, date of birth and school when you contact us.
When we receive your request, we will contact the Head Teacher and other professionals working with your child to ask whether they think the child needs an assessment. The Head Teacher will give us the information we need to decide if a statutory assessment is needed.
