If you wish to become a chaperone you will need to fill out a chaperone application form (
PDF 24K). This form must be filled in using black ink and returned with 3 passport size photographs.
Upon receipt of a completed application form, the references will be requested.
A member of the Access and Attendance Team will then arrange to visit you at your home address.
This visit will include a discussion about the role of the chaperones. You will also be asked to complete a Criminal Records Bureau form. Criminal Records Bureau forms are also known as Police check forms.
The Access and Attendance officer will also need to see your birth certificate, National Insurance number and, if applicable, your driving licence, passport and marriage certificate.
If the application is approved, the licence will be issued along with an Identity card and an acceptance letter.
Please note: There could be up to a 6-week waiting period for the Criminal Records Bureau form to be returned before the chaperone licence can be issued.
